29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) 35 Bonaire Circle, Suffern, NY (Exterior Front):  An employee was required to install roofing material on a 6 on 12 pitch roof and was exposed to a fall hazard of approximately 18ft to the asphalt below as a personal fall arrest harness was not secured by roof anchorage. Condition observed on or about 07/24/2024   Construction By Allison, Inc., was previously cited for a repeat violation of this Occupational Safety and Health Standard or its equivalent standard. 1926.501(b)(13), which was contained in OSHA inspection#1654989, 002, item number 001, issued on 3-29-23 and was affirmed as a final order on 6/30/22, with respect to a workplace located at 86 Division Ave, Spring Valley NY,10977.  In regard to Inspection Number #1695454 002, item number 001, issued on 9-19-23 with respect to a workplace located at 18 Buckingham Road, Nanuet NY 10954 and was placed in contest on 10/31/23.
