29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A.) The Tapco Tube Company, Tooling Area, On or about 4/24/24 and times prior thereto: guarding on machines did not prevent employees from contacting moving parts, chips, and coolant.  Machines include but are not limited to Trak DPM Serial # 96-2350.  B.) The Tapco Tube Company, Small Conveyor Laser 2, On or about 7/30/24 and times prior thereto: the Small Conveyor Laser 2 was not adequately guarded to prevent employees from contacting pinch points in that the guard was in the up position and the safety interlock was not connected.  Employee in the area when picking up tubes suffered an amputation to the right middle finger.    C.) The Tapco Tube Company, Jack Line, On or about 8/6/24 and times prior thereto: Moving parts and nip points were not protected on the back side of the conveyor where boxes are glued by a guard or device.   D.) The Tapco Tube Company, Red Line, On or about 8/6/24 and times prior thereto: the fixed guarding around the Robot did not totally enclose the robot area and working envelope. prevent employees from contacting moving parts. Moving parts include but are not limited to robotic arms.     ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement
