29 CFR  1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control  A.) The Tapco Tube Company, On or about 4/24/24 and times prior thereto:  the verification of isolation step in procedures for equipment was not specific. Equipment includes, but is not limited to, WH200 Wash/ Rinse System, WH200 Mill, Coil Cart, Shea/Endwelder, and Accumulator.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(c)(6)(ii):The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the ins   A.) The Tapco Tube Company, On or about 4/24/24 and times prior thereto: employees periodically conducted service, maintenance, and repairs on equipment. A Periodic inspection was being conducted of the general energy control procedure.  The certification did not include the date the inspection occurred.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

A.)The Tapco Tube Company, On or about 8/6/24 and times prior thereto: The affected lockout tagout training did not include:  1. Information on recognition of LOTO devices 2. The Energy control procedure  3. Information on not tampering with lockout devices 4. The definitions of a service/or maintenance task, authorized employee, affected employees, etc. are not included. These definitions would help an affected employee understand the LOTO program.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A.)The Tapco Tube Company, On or about 8/6/24 and times prior thereto:  Employees did not lockout prior to servicing operations on equipment including but not limited to, saws, endformers, and lasers. Employees are periodically required to conduct servicing operations including but not limited to, blade changes and tool changes.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(e)(3):Specific procedures and training that were developed, documented and incorporated into the employer's energy control program for removal of a lockout or tagout device under the direction of the employer when the authorized employee  A.) The Tapco Tube Company, On or about 4/24/24 and times prior thereto:  Procedures did not contain specific methods to be used to contact the employee to inform him/her that his/her lockout or tagout device has been removed.  The specifics on how the employee will be notified upon return to work if all contact methods fail was not noted in the procedure.      ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement
