29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at 7400 block of NW Pampas Ln, Parkville MO -- employees engaged in roofing duties using pneumatic nail guns, staplers, hammers, and other small hand tools were not wearing any eye protection. This exposed the employees to shrapnel and/or debris to get in their eyes causing injury. This was documented on or about 9/9/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a new residential worksite located at 7400 block of NW Pampas Ln, Parkville MO -- employees engaged in roofing duties were using an extension ladder to gain access to the roof. The ladder did not extend 3 feet above the landing surface (meaning the roof in this case). This exposed the employees to fall hazards greater than 10 feet to ground level. This was documented on or about 9/9/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a new residential worksite located at 7400 block of NW Pampas Ln, Parkville MO -- employees engaged in roofing duties were not using any fall protection system. This exposed the employees to fall hazards greater than 10' above ground level. This was documented on or about 9/9/2024.  Marvin Omar Mejia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1688327, citation number 1, item number 1 and was affirmed as a final order on 5/2/2023, with respect to a workplace located at 3900 W. 140th Dr. Leawood KS.
