29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 5507 E 130th Ave, Thornton CO: On or about October 8, 2024, and at times prior, employees were exposed to falls of approximately 10 feet to the ground below while accessing/egressing upper levels of the existing residential home. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 5507 E 130th Ave, Thornton CO: On or about October 8, 2024, and at times prior, employees were exposed to falls of approximately 10 feet to the ground while conducting roofing activities without fall protection. Valencia Roofing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.501(b)(13), which was contained in OSHA inspection number 1711968, citation number 1, item 1 and was affirmed as a final order on April 2, 2024, with respect to a workplace located at 2719 Garden Dr Fort Collins, CO 80526.
