29 CFR 1910.38(c)(6): The emergency action plan did not include the name or job title of every employee who may be contacted by employees who need more information about the plan or an explanation of their duties under the plan. On October 31st, 2024, and at times prior thereto, the AAFES written emergency action plan did not include the contact information for employees who may be contacted about the plan. Page 8 of the plan, "Building Emergency Contacts" was not filled out. In the event of an emergency, employees might not know what to do or how to do it, exposing them to potential hazards such as fire.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: Near the emergency exit by the dressing rooms, on October 22, 2024, and at times prior thereto, one fire extinguisher did not have an annual maintenance check within the past year. The date on the tag was 2018. In the event of an emergency, employees were potentially exposed to fire hazards which can cause burns or smoke inhalation.
