29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard: 5.1.6: Eyewashes shall be capable of delivering flushing fluid to the eyes not less than 1.5 liters per minute (0.4 gpm) for 15 minutes. If shut off valves are installed in the supply line for maintenance purposes, provisions shall be made to prevent unauthorized shut off.  Cleaning Room/Supply Storage: On or about November 6, 2024, and at times prior thereto, the employer failed to provide an emergency eye wash station, providing 15 minute flush time, where employees used corrosive chemicals, which potentially exposed employees to corneal or retinal burns.  The Army & Air Force Exchange Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR  1910.151(c), which was contained in OSHA inspection number 1630272, citation number 1, item number 1 and was affirmed as a final order on December 13, 2022, with respect to a workplace located at 10 Sonoma Ave., San Luis Obispo, California, 93405.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Retail Store: On or about November 6, 2024, and at times prior thereto, vendor refrigeration units, were plugged into a relocatable power tap and not used according to listing and labeling instructions. Employee(s) were exposed to potential electrical shock and/or fire hazard(s).  The Army & Air Force Exchange Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR  1910.303(b)(2), which was contained in OSHA inspection number 1446412, citation number 1, item number 5, and was affirmed as a final order on 01-15-2020, with respect to a workplace located at 461 Skymaster, Travis AFB, CA 94535.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Retail Store: On or about November 6, 2024, and at times prior thereto, an orange flexible cord was used as permanent power for an Air King Industrial Pedestal fan used to dry the retail store floors after mopping occurred.  This condition potentially exposed employee(s) to electrical shock and/or fire hazards.  The Army & Air Force Exchange Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR  1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1446412, citation number 1, item number 7, and was affirmed as a final order on 01-15-2020, with respect to a workplace located at 461 Skymaster, Travis AFB, CA 94535.

29 CFR  1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use:  Cleaning Room/Supply Storage: On or about November 6, 2024, and at times prior thereto, the employer did not have hard copies or operational electronic access to safety data sheets for hazardous chemicals known to be present and used by the employees, which potentially exposed employees to retinal or corneal burns.  The Army & Air Force Exchange Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.1200(g)(1), which was contained in OSHA inspection number 1596192, citation number 2, item number 2 and was affirmed as a final order on 07-19-2022, with respect to a workplace located at 5443 Dudley Blvd., McClellan, CA 95652.
