29 CFR 1910.219(c)(4)(i): Unguarded shaft ends projected more than one half the diameter of the shaft: Distribution Center: On January 24, 2025 and at times prior thereto, the shaft ends of the conveyor system were unguarded in the following areas: 1. Aisle 18, Module 18, with a height of approximately 64.5" above the floor 2. Security Area, Level 2 near shelf CPF 32 3. Conveyor near CPF 12 Module 11 on the 1st Floor Employees working as material handlers were potentially exposed to contact with rotating parts which can cause serious injuries.

Distribution Center near Aisle 18: The capacity sticker on one blue hopper attachment for a powered industrial truck was damaged and the capacity was not readable. This condition potentially exposed employees to powered industrial truck hazards such as dropped loads which can cause injuries.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. Maintenance room of the distribution center: On January 24, 2025 and at times prior thereto, one acetylene cylinder was stored next to one oxygen cylinder on a cart without a noncombustible barrier between the two. This condition potentially exposed employees to fire hazards.

29 CFR 1910.305(g)(1)(iv)(D): A flexible cord was used where attached to building surfaces. Distribution Center near Aisle 18 and Module 18: On January 24, 2025 and at times prior thereto, the cord to a portable fan was attached to building surfaces including the conveyor module, structural steel beam, and the exterior of electrical conduit. Connecting an appliance cord to building surfaces can cause damage to the cord. This condition potentially exposed employees to an electrical hazard.
