29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:  Front side of the building interior: Employees used a stairway rising over 11 feet that was not equipped with a stair rail system on its unprotected side. Employees were exposed to fall hazards.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.  Second floor Balcony: Employees used a Bosch Bulldog Xtreme max chipping hammer to chip concrete without any control measures implemented from Table 1-Specified Exposure Control Methods When Working with Materials Containing Crystalline Silica. Employees were exposed to respiratory hazards.

Li Gao Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1703240, citation number 1, item number 1 and was affirmed as a final order on 12-12-2023, with respect to a workplace located at 223-299 Chalan Padiron Lagu, Yigo GU, 96929.

29 CFR  1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. 29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the Electrical Box: The outer insulation of the extension cord plugged into the outlet was pulled back exposing the conductors to strain. Employees were exposed to electrical hazards.
