1910.212(a)(3)(iv)(a) Guillotine cutters. (a) Carrot processing line; On or about October 31, 2024, and dates prior, the rotating bowl (separator) section of cutter machines was not guarded. On October 28, 2024, an employee suffered a partial finger amputation while reaching into the bowl section during production operations to clear a stuck carrot.

On or about October 31, 2024, the employer did not provide a description to identify the object or substance that directly caused the injury (column F) on the OSHA Form 300 or equivalent for calendar year 2024. (a) On or about October 28, 2024, an employee working on the Root crop Mezzanine sustained a lacerated fingertip that was medically defined as an amputation, case no. UH-3. The object or substance that directly injured the person and specific finger(s) injured was not described in column F. (b) On or about September 25, 2024, an employee working on the label line sustained a back strain injury, case no. UH-2. The object or substance that directly injured the person and specific area of the back injured was not described in column F.
