29 CFR 1910.132(a): Protective equipment was not used, when necessary, whenever hazards capable of causing injury and impairment were encountered: (a) Facility, Pouring Floor: On or about January 17, 2025, four employees were pouring molten metal (magnesium) into molds and did not have any neck nor back of the head protection from molten metal (magnesium) splashes. (b) Facility, Furnace Operator Area: On or about January 17, 2025, the furnace operator, who was wearing no personal protective equipment, dumped alloys from a 5-gallon bucket into a furnace of molten magnesium and this condition resulted in molten magnesium being thrown out of the furnace by about 8 feet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 17, 2025, the nip points created by the powered conveyor belt and the steel rollers were not protected from employee contact. These conditions created pinch points to employees. (b) On or about January 17, 2025, employees were not protected by the rotating motion of the Kostler Mold Machine that would turn molds upside down. (c) On or about January 17, 2025, the point of operation on the 200-ton press was not guarded against employee contact.
