29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: Building 64 Shipping Department: On or about December 3, 2024 and at times prior thereto, industrial storage racks in the building were free standing and not anchored to the floor. Employees who retrieve items from these storage racks, as well as employees working in the immediate vicinity, were potentially exposed to struck-by and/or crushing hazards if the storage racks were to fall or be tipped over.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. Building 64 Break Room: A Keurig coffee machine was plugged into a relocatable power tap (RPT), also commonly referred to as a power strip. Plugging higher amperage equipment such as small appliances into an RPT can lead to overheating, electric shock, and fires. Employees were potentially exposed to electrical hazards.
