29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the jobsite located at 19650 W. 201st St. Spring Hill, Kansas, employees engaged in residential  framing activities  were exposed to fall hazards of approximately 15 feet 10 inches above the ground. Fall protection was on site but not utilized.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 19650 W. 201st St. Spring Hill, Kansas, employees engaged in residential framing activities were exposed to fall hazards in the following instances:  a) On the southeast corner of the partially framed residential structure, employees utilized Werner 16 ft. extension ladder. The extension ladder did not extend three-feet above the upper landing surface b) On the north side of the partially framed residential structure, employees utilized K-Pro 8 ft. A-Frame . The A-Frame ladder was leaned up against the north side of the partially framed residential structure, it was not used according to manufactures guidelines for use.
