On or about February 10, 2025, employees were engaged in residential roofing activities, located at 5411 N 181 Ave, Elkhorn, NE 68022. Employee(s) are exposed to struck-by-hazards in that the operator vehicle was not evaluated prior to operating the powered industrial truck.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about February 10, 2025, employees engaged in residential roofing activities, located at 5411 N 181 Ave, Elkhorn, NE 68022. Employee(s) were exposed to fall hazards when working in excess of 6-feet on a residential roof without fall protection. Beltran Framing was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) which was contained in OSHA inspection number 1575798, citation number 1, item number 4 and was affirmed as a final order on May 26, 2022 with respect to a workplace located at 16930 Nebraska Ave., Omaha, Nebraska 68116. Beltran Framing was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) which was contained in OSHA inspection number 1556193, citation number 2, item number 3 and was affirmed as a final order on January 25, 2022, with respect to a workplace located at 8424 Sheridan Street, Omaha, NE 68122.
