29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, employees installing a roof on a residential home roof with a 9:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 12 feet 11 inches from the roof eve to the ground below on the front (south) side of the home.

(a) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, the employer did not ensure employees conducting installation of a new residential roof, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.  Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1629767, citation number 1, item number 3 and was affirmed as a final order on April 28, 2023, with respect to a workplace located at 414 S. Silver Lake St., Oconomowoc, WI 53066.

(a) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.  Everardo Servin LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1621869, citation number 2, item number 2 and was affirmed as a final order on January 12, 2023, with respect to a workplace located at 1236 Allen Rd., Oconomowoc, WI 53066. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110,  employees were accessing the roof to conduct roofing work from a ladder on the left (west side) of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 6 inches.  (b) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110,  employees were accessing the roof to conduct roofing work from a ladder on the front (south) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately less than 1ft past upper landing), exposing employees to a fall hazard of greater than 6ft.  (c) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110,  employees were accessing the roof to conduct roofing work from a ladder on the right (east) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 9 inches.  (d) On or about March 14, 2025, at 3330 E. Grange Ave., Cudahy, WI 53110,  employees were accessing the roof to conduct roofing work from a ladder on the right (east) side of the home which did not extend at least three (3) feet above the upper landing surface (extended approximately less than 1ft past upper landing), exposing employees to a fall hazard of approximately 12 feet 9 inches.
