29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about March 26, 2025, at 10330 S. Nicholson Rd., Oak Creek, WI 53154, employees conducting roofing activities on a 10:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was located in the front and centered at the waist (belt area) not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to a fall hazard of approximately 10 feet 4 inches on the right (south) side of the home.
