29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)Cardona Construction at 1720 South Southeastern Avenue, Sioux Falls, SD 57103:  On or about and at times prior to May 28, 2025, the employer did not ensure that frequent and regular inspections were conducted by a competent person. This condition exposed employees to various hazards such as fall hazards, personal protective equipment (PPE) hazards, and a lack of training.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a)Cardona Construction at 1720 South Southeastern Avenue, Sioux Falls, SD 57103:  On or about and at times prior to May 28, 2025, the exposing employer did not ensure that employees using personal fall arrest systems had them rigged in a manner that prevented a free fall or contact with a lower level of greater than 6 feet. This condition exposed employees to fall hazards greater than 6 feet.

29 CFR  1926.501(b)(10): Employees engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:   (a)Cardona Construction at 1720 South Southeastern Avenue, Sioux Falls, SD 57103:  On or about and at times prior to May 28, 2025, the exposing employer did not ensure that employees were protected from falls while conducting roofing operations on a low-slope commercial property.  This condition exposed employees to fall hazards greater than 6 feet.  Cardona Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1495372, citation number 1, item number 1a and was affirmed as a final order on November 25, 2020, with respect to a workplace located at 1000 East Teakwood Street, Brandon, SD 57005.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)Cardona Construction at 1720 South Southeastern Avenue, Sioux Falls, SD 57103: On or about and at times prior to May 28, 2025, the employer did not ensure that fall protection training was documented by a written certification prior to employees being exposed to fall hazards. This condition exposed employees to fall hazards greater than six feet.  Cardona Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1495372, citation number 1, item number 1b and was affirmed as a final order on November 25, 2020, with respect to a workplace located at 1000 East Teakwood Street, Brandon, SD 57005.
