29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a) Jobsite located at or near Vienna Blvd, Lot 73-69, West Brandywine, PA 19343 - On or about June 3, 2025, the employer did not ensure that employees working on an elevated platform were adequately attached to the forks. Employees were exposed to fall hazards while occupying the platform during its movement.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at or near Vienna Blvd, Lot 73-69, West Brandywine, PA 19343 - On or about June 3, 2025, the employer did not provide fall protection training to employees who were exposed to fall hazards.  Employees were performing framing activities from heights greater than 6 feet without being trained on fall protection, thereby exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Jobsite located at or near Vienna Blvd, Lot 73-69, West Brandywine, PA 19343-  The employer did not ensure that the ladder used by employees to access the upper landing surface of the roof was extended to at least 3 feet. Employees were exposed to fall hazards approximately of 10 feet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite located at or near Vienna Blvd, Lot 73-69, West Brandywine, PA 19343 - On or about June 3, 2025, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected guardrail systems, safety net system, or personal fall arrest system. Employees were performing framing on top of the first and second  story without the use of fall protection, thereby exposing employees to falls of approximately 10-30 feet.  Lake Contractor Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1774989, citation number 1, item number 1 and was affirmed as a final order on March 14, 2025, with respect to a workplace located at 18 Sofia Dr., Manalapan, NJ.  Lake Contractor Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1781109, citation number 1, item number 2 and was affirmed as a final order on March 28, 2025, with respect to a workplace located at 450 Commerce Lane, Berlin, NJ.
