On or about July 7, 2025, at the location of 480 Osterloh Rd. Minster, OH: The employer did not ensure employees tasked with handling structural steel components (2 ton I-beam) on the "Wheelabrator's" west powered roller conveyor, properly cleared the lifting clamps from the I-beam prior to moving the crane and/or crane hoist, exposing employees to crushing hazards. Among other methods, feasible and acceptable means to correct this hazard would be to follow the guidance in American Institute of Steel Construction (AISC) 2004 "General Safety Rules for Steel Fabricators" Section 3: Floor Operated Cranes and Hoists.

Kard Welding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.219(f)(3), which was contained in OSHA inspection number 1513816, citation number 1, item number 2a and was affirmed as a final order on July 28, 2021, with respect to a workplace located at 480 Osterloh Rd.
