29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) On or about August 26, 2025, at the above addressed jobsite, employees performing residential framing operations seventeen (17) feet above the ground, used lifeline lanyards that were installed around a beam and connected to themselves, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about August 26, 2025, at the above addressed jobsite, employees performing residential framing operations, seventeen (17') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards. TBR Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1735300, citation number 2, item number 1 and was affirmed as a final order on July 7, 2024, with respect to a workplace located at 6014 Fairview Avenue in Downers Grove, Illinois 60516. TBR Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1711272, citation number 2, item number 2 and was affirmed as a final order on February 28, 2024, with respect to a workplace located at 12283 Copper Ridge Court in Lemont, Illinois.
