29 CFR 1910.253(b)(4)(iii):Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. a) On or about September 5, 2025, the employer did not ensure oxygen tanks and propylene tanks in storage next to welding bay 2 were separated by a minimum of 20ft or by a noncombustible barrier.

29 CFR 1910.1200(h)(1):Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. a) On or about September 5, 2025, the employer did not train employees on the hazards of chemicals such as, but not limited to, coolant (skin irritation, eye damage) and WD-40 (respiratory irritation, skin irritation).
