29 CFR 1910.178(n)(8): Powered industrial trucks. Traveling. Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner. a) On or November 11, 2025, the employer failed to ensure that powered industrial trucks were operated at a speed that permitted them to be brought to a stop in a safe manner. In the outside yard area by Building #4, a forklift was traveling at a speed that prevented it from stopping safely and struck an employee, causing the load to fall. As a result, employees were exposed to struck-by and crushing hazards resulting in an injury.

29 CFR 1910.178(n)(4): Powered industrial trucks. Traveling. The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. a) On or about November 11, 2025, the employer failed to ensure that powered industrial truck operators slowed down and sounded the horn at intersections where vision was obstructed. An employee operating a forklift in the yard continued driving with a load of lumber in front, while the doorway of Building 4 was visibly obstructed by two parked trailers. As a result, a pedestrian using the marked walkway was struck by the forklift and injured.
