29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: Instance A: 325 State Rt. 31, Building K, Montgomery, IL 60538: On or about November 29, 2025, employees were working from an unprotected edge more than 4 feet from the ground while conducting maintenance to the Vecoplan grinder. The employer did not ensure that each employee on a walking working surface with an unprotected side or edge that is 4 feet or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i). Instance B: 325 State Rt. 31, Building K, Montgomery, IL 60538: On or about November 30, 2025, employees were working from an unprotected edge more than 4 feet from the ground while conducting maintenance to the Vecoplan grinder. The employer did not ensure that each employee on a walking working surface with an unprotected side or edge that is 4 feet or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i). Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).

29 CFR 1910.30(a)(1):The employer did not provide training for each employee exposed to fall hazards and who used personal fall protection systems or is required to be trained as specified elsewhere in this subpart prior to May 17, 2017: Instance A: 325 State Rt. 31, Building K, Montgomery, IL 60538: On or about November 29, 2025, employees were providing maintenance to the Vecoplan grinder, exposing employees to a fall of more than 4 feet from the ground. The employer did not provide training for each employee on the hazards associated with working from elevated levels before any employee was exposed to a fall hazard. Instance B: 325 State Rt. 31, Building K, Montgomery, IL 60538: On or about November 30, 2025, employees were providing maintenance to the Vecoplan grinder, exposing employees to a fall of more than 4 feet from the ground. The employer did not provide training for each employee on the hazards associated with working from elevated levels before any employee was exposed to a fall hazard. Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).
