29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) On or about December 8, 2025, the abrasive wheel on the company's Belt Grinder in the Small Grind Area was worn leaving approximately one inch between the wheel and the work rest. The operator and/other employees working in the immediate area were potentially exposed to flying projectiles and/or shards of the abrasive wheel and other materials if the wheel were to break or material got caught between the frame and work rest.
