On [REDACTED] at approximately [REDACTED] 1 received a call from the picking supervisor concerning an athlete that had an earlier incident and wanting to know what their next steps were. The athlete stated that their left shoulder was hurt and they could hardly move their arm as a result of a fall approximately [REDACTED] to [REDACTED]. EHS 1 was able to find the incident in question and it occurred [REDACTED] and there was no mention of an arm or shoulder injury. EHS 1 asked the athlete if they mentioned that their shoulder was injured to anyone the day of the incident or in the [REDACTED]. The athlete stated no. EHS 1 told them to return to their supervisor and they would be instructed what to do next. EHS 1 brought the incident to the attention of the general manager and returned to the safety office. At approximately [REDACTED]. The athlete returned to the safety office at the direction of the GM to be sent to Concentra to be checked out.
