I was with another coworker placing Z foam under a 70 lb female patient in TCU room #[REDACTED].

While pushing the foam under the patient I felt a pop and then pain in my left shoulder. Despite treating it with [REDACTED] for the [REDACTED] there has been no improvement. The shoulder feels as if it is popping slightly out of place at times and I have to manually put it back in place. The last time this happened was while I was on the toilet leaning forward with my elbows on my knees. I had to push my shoulder against the corner of the wall to get it to return to the correct position. I deny numbness tingling or radiating pain but my arm feels heavy and achy when hanging at my side.

The injury involved a possible dislocation or subluxation of the left shoulder. Specifically the shoulder feels as if it is popping out of place and requires manual adjustment to return to the correct position. The arm feels heavy and achy when hanging at my side.

The object that directly harmed the employee was the Z foam being placed under the patient. The physical effort required to push the foam under the patient caused the injury to my left shoulder.”

The left shoulder was strained or tear or subluxated due to pushing Z foam under a patient.
