I was giving report to [REDACTED]. We helped a patient to the bathroom and I went to turn off the call light on the wall.

While turning off the call light the recliner handle caught my foot. The room is small and I tripped and fell to the floor on my knees twisting my ankle.

The injury involved a sprain in the ankle and bruising on both knees. Specifically the fall caused a sprain in my ankle and bruising on my right and left knees.

Fall from same level. The objects that directly harmed the employee were the recliner and the floor. The recliner handle caused the trip and the impact with the floor caused the injuries to my ankle and knees.

The ankle was sprained and both knees were bruised due to tripping over a recliner handle and falling to the floor.
