Pulling away from charging station on a forklift

As a forklift driver was returning from break he forgot to unplug his lift before turning it on. The employee released the parking break and began to travel forward. This resulted in the charger being pulled from the wall damaging the power cord and 480V power feeding the disconnect. The employee stated the jerk from the incident resulted in a neck injury that radiated down his shoulder and back. The employee was evaluated at an urgent care clinic and received prescription medication and restrictions
