What happened

At approximately [REDACTED] on [REDACTED] a deco operator had completed adding ink to inker number 6 on Decorator 1B. A water line broke loose spraying the employee in the face and wetting the platform. The employee slipped forward falling into the side of the decorator. The employee hit the right side of their face with visible redness swelling above their right eyebrow and nose. The employee had their hair hanging down in two braids. When the employee fell their left braid was caught in a rotating shaft and pulled from their head. The employee pulled the e-stop to stop the machine. The employee’s scalp remained intact with superficial abrasions to their head and face. The employee was seen at the Medical Center Urgent Care for evaluation. The employee was given 2 prescription medications. The employee was given the restrictions of no lifting of 5lbs no overhead work and no work at or below chest level. The employee was released to return to work on [REDACTED].
