Before the incident
On [REDACTED] at approximately [REDACTED] an EE with [REDACTED] of tenure was walking into the building when they slipped and fell striking their left knee and left face on an EPJ that was parked near the entrance It was raining outside at the time of the injury The EE entered the entrance for the start of their shift where there was a rubber mat for EEs to wipe off their shoes EE walked across the mat and while walking around the forks of a parked EPJ their right foot slipped out from under them causing them to fall into the EPJ The EEs left side of their face struck a pallet that is used as a backstop on the EPJs and their knee struck the corner of one of the forks After speaking with the employee and reviewing the video the EE stated there was no water on the ground prior to them walking in and that the water was residual on the bottoms of their shoes from walking in the rain EE states they normally stop and wipe their shoes but they felt rushed as they were running later than they
