On [REDACTED] at approximately [REDACTED] an injury occurred in the yard of the facility EE DOH ([REDACTED]) was in the spotter function EE was at [REDACTED] As EE began to walk to remove jackstand EE slipped and fell When EE got up they fell again in the process EE hurt their right wrist and lower back On [REDACTED] EE went to Urgent care and was diagnosed with strain of muscle and tendon of back wall strain of muscle fascia and tendon of lower back other muscle spasm and contusion of right front wall of thorax EE was given prescription medication and restrictions until [REDACTED] and was instructed to return to clinic for a recheck on [REDACTED]
