Before the incident
At approximately [REDACTED] [REDACTED] [REDACTED] contractor employee was performing routine inventory spot checks and detrashing material in bulk rack aisle 4 He was working at ground level on the cherry picker levels 1 and 2 He noticed a part that needed attention at the third level and decided to address the part before grabbing a harness to elevate himself to the third level While elevated at roughly 5 feet he lost his footing when turning and fell into one of the forks on his back He then fell to the ground on his feet He got back into the truck and lowered it back to ground level then continued to work There were no direct witnesses but one contractor employee heard some commotion in the next aisle and decided to check in on employee They said theyre fine and continued working [REDACTED] employee went to their lead and explained what happened and that they felt some soreness and would like to go home Lead immediately told manager but employee had already left Employee has not ret
