Before the incident
On [REDACTED] at approximately [REDACTED] a bread employee who was working in wrap on line 1 suffered an injury to their right hand Employee saw a bread bag was stuck on machinery scoops and upon reaching in to grab the bag was struck by the edge of the top metal scoop resulting in roughly a 1inch laceration between right thumb and right index finger Employee notified supervisor and received first aid care Outside medical was called and employee was sent out for further treatment Employee received 6 stitches and restrictions Employee originally stated they had followed proper procedure of turning off machinery before reaching in but after reviewing video footage this was proven to be incorrect Employee had not turned off machinery and had instead reached under protective hood while machine was active Employee has a tenure of 7 years and had been operating in bread wrap for almost 4 years There was not a rushing component or any environmental factors that contributed to this incident Em
