What happened
Employees were tasked with removing convection tubes from F 14 Furnace pre heater in [REDACTED] 1 using a tugger beam crane and forklift A tube had been extracted and was on the tugger beam The tugger beam was then lowered to the ground for the crew to remove the tube While the tube was being extracted from the beam using the forklift and rigging the tugger operator called for the forklift to stop so the tugger end of the rigging could be removed When the injured employee reached in to remove the rigging from the tube the tube shifted towards the forklift causing tension on the cable resulting in pinching the employees right middle and ring fingertips between the tugger cable and snatch block sheave
