Employee was performing her duty as a fish packer - sorting packing fish into proper size styrofoam containers.

Worker was performing her duty according to witness when a 2-3 size stone of size fell off the cooling unit above and hit her. One witness claimed the stone hit her on the shoulder the witness and [REDACTED] laughed about it. Then [REDACTED] the fresh area manager inquired to ask what happened and [REDACTED] claimed it hit her in the head. [REDACTED] also a certified first aid responder assessed her for no visible concussion no sign of blood bruise or cuts. Offered her ice and rest suggested arranging travel to ER to get assessed by a doctor. [REDACTED] declined and said she would go with her agency.

Inconclusive injury due to lack of information from hiring agency. Due to HIPPA we never received full documentation of injury. A note was sent to us from hiring agency saying she was seen and treated for something on 10 8 then again on 1 14 under going therapy. When we inquired about a note for our investigation or light duty the hiring agency never confirmed visit to be work related determination of diagnosis or a return to work note. Hiring agency cleared her for work on 10 28 without doctor

A 2-3 Piece of ice fell from cooling unit onto the worker maintenance called a technician to assess the cooling unit the next day following as corrective action.
