Before the incident

The employee was cleaning the floors by line 3 in production on the value added cutting side. Particularly under around tables & pallets with a squeegee broom. The employee was bent over cleaning with the squeegee broom before the injury.

What happened

After reviewing camera footage we saw the employee leaning on the squeegee pushing material and then falling to the ground. Unfortunately the camera angle does not show the cause of the fall. First aid tried to communicate questions but [REDACTED] did not answer them at the time. According to AV Temp Agency [REDACTED] reported that he lost his footing and did not catch himself since he was holding the squeegee. He was not dizzy tired or ill that day.

Injury or illness

Laceration above eye due to direct contact with the floor during his fall. Laceration greater than 1 8 needed stitches & precautionary scans. He was wearing glasses that broke during the fall we could not determine if the glasses caused the laceration or the floor itself.
