Before the incident
The employee was moving pallets with a forklift in the freezer when he got off of the forklift to clean a spot on the floor to apply tape and fell.
What happened
When the employee got off of the forklift to clean a spot on the floor to apply tape he slipped on an ice patch. (This ice was caused by temporary work due to replacement of new freezer doors. The warmer air caused the moisture to freeze. [REDACTED] fell on his upper back shoulder and possibly head he was evaluated at the ER there was no concussion mild soreness.
