Cleaning the sensor
What happened
Glass was exiting the Glass Butterfly machine in a misaligned manner. The operator decided to clean a sensor on the Butterfly conveyor by hand. In order to access they stood on top of the mid rail of the surrounding guardrail reaching out and over to the sensor with their right hand. In doing so the operator bypassed the interlocked gate and was elevated off of the safety mat which would have shut down the machine. As the operator was wiping the sensor their hand came in contact with the moving conveyor drive chain located directly next to the sensor. The chain pulled the employees right index finger causing deep lacerations and an open fracture.
