What happened

On [REDACTED] at approximately [REDACTED] the EE reported that [REDACTED] ([REDACTED]) he smashed his thumb. The EE stated that the reason he smashed his finger is because the pallets were stacked incorrectly and when he went to sit down the pallet on his walkie-rider his thumb was caught in between pallet and walkie-rider. The EE was offered to be evaluated by [REDACTED] for his finger but declined seeing [REDACTED]. The EE stated they were immediately going to the clinic to be examined. According to some witnesses who was working the same [REDACTED] the incident supposedly occurred they did not know an incident occurred and EE had been working normally for the [REDACTED] until he reported the injury. On [REDACTED] Safety Supervisor asked the EE to go and show us exactly how the incident occurred and EE said “I am not sure how incident occurred”. EE then paused for a moment and said “Okay I guess I can go show you how it occurred”. The EE was diagnosed with 10lb restrictions.
