Incident Report – Powered Industrial Truck (PIT) Contact Injury Employee: [REDACTED] Date of Incident: [REDACTED] Time of Incident: [REDACTED] Location: Battery Charging Area Incident Description: At [REDACTED] on [REDACTED] [REDACTED] was parking his walkie when he accidentally pinched his ankle against a yellow barrier. At the time of the incident the employee was off the MHE (Material Handling Equipment) and decided to move the walkie forward which caused the walkie to press his ankle against the barrier. [REDACTED] immediately repositioned the walkie to relieve the pressure. The right ankle appeared slightly swollen and showed signs of a scrape. First aid was administered on-site including the application of pain relief gel gauze and an ice pack. The team member declined immediate medical attention through Medcor and opted to complete is shift. [REDACTED] at approximately [REDACTED] on [REDACTED] [REDACTED] returned to work for his shift.
