Incident Summary – [REDACTED] On [REDACTED] at approximately [REDACTED] [REDACTED] reported experiencing severe neck pain and intense discomfort. She informed her supervisor that the pain began while she was performing the start-up stretching routine. First aid was immediately offered but [REDACTED] declined to contact Medcor. It is important to note that [REDACTED] had previously stated to the department lead that the discomfort “did not happen inside Ryder.” On [REDACTED] our Onsite Human Resources Business Partner followed up with [REDACTED] in response to a message she left about seeking medical attention. During the call [REDACTED] indicated that she was still in pain and wished to be seen by a physician. She was provided the address for our Concentra Clinic. At Concentra she was diagnosed with acute strain of the neck muscle – initial encounter.
