Floor was slick where employee fell. Normally there is a mobile creel located at location of fall. The mobile creel had been pushed out by the last crew but there was not one available to replace it. This area of the floor is not normally used for walking and there was binder build up along with it being wet.

Employee was working on [REDACTED] North side winding level. She had just doffed a package off and put it on the table. She was walking towards the big washdown hose located behind [REDACTED] [REDACTED]. She walked slightly by [REDACTED] [REDACTED] into the area between [REDACTED] and [REDACTED]. Her right foot slipped causing her to fall backwards and to the left landing on her left hip just in front of a dumpster beside the I-beam at [REDACTED] [REDACTED]. She remained on the floor and called to the other doffer working beside of her. He hurried over to her. Realizing that she needed medical assistance he went around to the south side of [REDACTED] then out in the main hallway looking for someone with a radio. Once he found someone EMT was notified and was there within 2 minutes of incident along with a host of others. Situation was assessed employee was carefully loaded onto a backboard and taken to medical services. A call was placed to EMS for an ambulance to transport to [REDACTE

Employee was taken to CRMC for evaluation. X rays revealed no fractures but left hip suffered contusion. Also left ankle was twisted and sprained.
