Picking cases of wine onto the conveyor.

On [REDACTED] at approximately [REDACTED] an employee working as a Picker reported an injury to their right lower leg due to being struck by broken glass after bottles fell out of an open case. The employee was working at the Picking horseshoe in front of a pallet of Prosecco when the incident occurred. The employee stated they were picking cases of Prosecco using the Vaccuhoist when they attempted to lift the third or fourth case the bottom of the case opened and the bottles fell out and onto the floor. The employee stated that the environment they were working in was normal dry hot humid and clean. The employee is qualified to perform the operation and has been trained in the process which is not new. The employee was using the appropriate tools for this process which is a Vaccuhoist. During the investigation it was discovered that the case the employee was attempting to lift was damaged either due to the high humidity caused by the weather or damage caused during transport (because
