Immediately prior to the incident the employee was involved in a maintenance function on the (X1HAM) [REDACTED].

What happened

As the employee was exiting the (open-grid flow-through) serrated steel platform surrounding the Hydromat he stepped onto the step below. The employee believes that he stepped down onto the tread nosing (where the tread and riser meet). There was a considerable amount of oil that had accumulated on the tread nosing and bottom the employee 's work boot. The employee 's foot placement and the oil caused the employee 's foot to slide forward and he began to fall. The employee was using the handrail while exiting the platform. As he fell the employee maintained his (right hand) grip on the handrail and extended his left hand to break his fall. The employee 's left hand made contact with the top of the platform during the fall. As a result of the contact the employee sustained a laceration to the outer palm of his left hand.
