Incident DescriptionWhile unloading glass at the offload end of [REDACTED] Station 2 the injured employee was preparing to handle the next batch which included oversized glass panels measuring 68 x 67 and 47mm thick from schedule 47804 Due to their size and weight these panels required a twoperson liftAlthough the supervisor was present and had offered assistance the injured employee declined and attempted to lift one of the oversized glass pieces alone During the lift the glass broke and a shard struck the employees right cheek causing a lacerationImmediate ResponseThe supervisor who witnessed the incident immediately assessed the employees injury and determined that first aid was necessary The supervisor promptly notified the shift manager and escorted the employee to the first aid room for evaluation Upon assessment it was determined that the injury required additional medical attentionMedical TreatmentA Cardinal first responder transported the employee to the Emergency Room at HCA
