What happened

On [REDACTED] at approximately [REDACTED] leadership was called to the [REDACTED] racking area for a seasonal associate doing their PIV training practical. They were reporting an injury due to making contact with a rack. The associate verbally stated that they were driving in the area in front of aisle L-09 and while they were driving they smashed their foot between the rack and the electric pallet jack. During the investigation the site found that the incident occurred during the associate’s PIV training practical of the EPJ. The associate was attempting to complete figure 8s around the set cones while driving with forks back and attempting a right turn. The associate was not able to complete the turn fully but instead started driving closer to the racks. Upon trying to correct the turn the associate then accelerated into the racking.
