It is assumed the employee was driving order picker and was trying to go down aisle HL in the tunnel and found it blocked by another order picker. Employee backed up to go down aisle HM.

Employee was driving order picker and was trying to go down aisle [REDACTED] in the tunnel and found it blocked by another order picker. Employee backed up to go down aisle [REDACTED]. The tunnel zigzags in aisles [REDACTED]-[REDACTED]. When the employee backed up in [REDACTED] tunnel there was no pallet or skid in the floor location. Employee had looked back once at floor for to confirm he was in the tunnel and free of obstacles then looked forward while backing up. Employee then misjudges location of the tunnel and backs into a cross beam. Pinning head between beam and order picker window guard. After striking beam employee drove himself to the nearest bathroom.
