What happened

On [REDACTED] at approximately [REDACTED] a Wayfair associate working in MM reported 6 pain level in their neck. The item (CS567463011-2 - 55 - 60 W x 76 H Double Sliding Frameless Shower Door) that the associate was attempting to offload weighed approximately 150lbs. The associate proceeded to place the item onto their dolly where the box was off centered and it fell towards them. The associate then pushed the box off them. After placing the item the associate had reported it to Team lead and the team lead instructed the associate to report the incident to the supervisor. Associate then reported the incident to the supervisor. After reporting their injury the associate was not able to return to their process path. The associate reached out to CORVEL for medical treatment.
