On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] was involved in an incident that occurred near [REDACTED] 915 on the shipping dock. The incident took place while the Outfitter was building a pallet. According to initial reports the incident involved the Outfitter attempting to lift a grill that was estimated to be over 50 pounds with no assistance. The Outfitter stated he felt pain in his lower back when he lifted the grill. [REDACTED] stated once he injured himself he did not immediately notify his supervisor because he felt like the injury was his fault. On [REDACTED] at approximately [REDACTED] the Outfitter stated he asked his supervisor [REDACTED] for pain medication for his back but did not disclose the nature of his injury and continued work.
