removing a wooden Chep pallet from the top of a pallet stack

On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as an unloader was involved in an incident that occurred at dock [REDACTED] 26 in the receiving area. The incident took place while the outfitter was removing a wooden Chep pallet from the top of a pallet stack. According to initial reports the incident involved Outfitter [REDACTED] pulling the top pallet by the corners closets to him and then releasing the pallet to fall on the floor. Outfitter [REDACTED] attempted to move the pallet onto the floor without steering clear of the falling pallet. As a result of the incident the outfitter sustained a bruised and swollen right toe a discolored toenail and sharp pain from the right big toe to the right ankle. Response at approximately [REDACTED] included first aid followed by a call to CorVel. At the start of reporting Outfitter [REDACTED] stated his pain was an 8 10.
